Planning for the future means deciding what will happen to your pension when you pass away. This ensures your wishes are carried out and your loved ones are protected. Share this page with your spouse, common-law partner, or beneficiary so they know what to do.

Death of a retired member 

Online: Use the Death Reporting Form

A copy or picture of the Certificate of Death or Funeral Director’s StatementNeeded in all cases.
Notarial copy of Will, Grant of Probate or Letters of AdministrationMay be needed if pension or insurance benefits are payable to an estate. CSSB will let the executor or administrator know if this is required. 
Please confirm with CSSB that you are entitled to these benefits before completing these forms:
Direction for Payment formNeeded if a monthly pension is to continue to a survivor or other beneficiary. 
Federal and Provincial TD1 tax formsNeeded if a monthly pension is to continue to a survivor or other beneficiary. 
Claimant’s StatementNeeded if there is a named beneficiary or beneficiaries for processing life insurance. 

Additional documents may be needed depending on the pension option selected by the member.

Whether monthly pension benefits continue after a retired member’s death depends on the option chosen at retirement:

Survivor option: Benefits may continue if the member predeceases their spouse or partner.

Guarantee option: Benefits may continue to a named beneficiary if the guarantee period hasn’t ended.

Death of a member 

If a married spouse survives you, CSSB needs the following:

  • Copy of marriage certificate
  • Copy of death certificate or funeral director’s statement
  • Copy of member’s proof of age
  • Copy of spouse’s proof of age

Proof of age is a copy of one of the following: 

  • Birth certificate 
  • Valid Canadian passport 
  • Valid driver’s license 
  • Canadian citizenship card 
  • Permanent resident card

A common-law partner is either

  1. A person who registered a common-law relationship under Section 13.1 of The Vital Statistics Act
    or
  2. A person who lived in a conjugal relationship without being married
  1. for at least three years, if either partner is married
    or
  2. for at least one year, if neither partner is married

Common-law partner documentation

CSSB needs to confirm if a common-law partner can receive benefits. If the relationship was registered with Vital Statistics, provide a copy of the certificate.

If the relationship wasn’t registered, CSSB needs:

  1. Two affidavits from family or friends confirming details such as the length of time the couple lived together and whether either partner was married before.
  2. Two documents from this list:
    • Joint bank account documents 
    • Joint mortgage agreement
    • Joint rental agreement
    • Joint credit card statements
    • Joint income tax returns
    • Joint health coverage 
  3. All of these:
    • A declaration form stating the length of time the couple lived together and whether either party was married
    • Copy of member’s death certificate or funeral director’s statement
    • Copy of member’s proof of age
    • Copy of partner’s proof of age

If you are not survived by a spouse or common-law partner, CSSB will need the following:

  • Marital Common-Law Relationship Status form
  • Copy of member’s proof of death
  • Copy of member’s proof of age

CSSB may also need:

  • Notarial copy of a Will (may be subject to Probate) or Letters of Administration 

If your spouse or common-law partner has waived their right to the pension benefits or if you don’t have an eligible spouse or partner, you can name a beneficiary with: 

  • A pre-retirement beneficiary designation form  
  • A handwritten, signed letter 
  • A pre-retirement beneficiary form signed by a Power of Attorney (must include the Power of Attorney document)
  • A Will
