Retirement Forms Supporting Documentation

Forms for application for a monthly pension are available by contacting the Board office, or eligible members may complete their retirement forms online when signed onto their CSSB Online Services account. The following forms are supporting documentation that may also be useful or required when applying for a pension.

Beneficiary Designation Form (only to be used if selecting a minimum guarantee pension at retirement)

Form 5A – Waiver of 60% Joint Survivor Pension

Form 5B – Consent For Integrated Pension

TD1 – Personal Tax Credits Return

TD1MB – Manitoba Personal Tax Credits Return

TD1 – Non-Manitoba Personal Tax Credits Returns
